New Patients

Steps to take before your first visit to S.E.E.D.S. of the Willistons:

  1. Please complete the appropriate PDF intake form and submit it to us before your first visit.

  2. Read the HIPAA Privacy Rules, print a copy for your records and submit the Insurance Form.

  3. Please read, agree and submit the Office Policy Agreement. Print a copy for your records. 

  4. If you plan to bill or get reimbursement from your insurance company, please check your coverage.
